ISBAR (Deteriorating Patient)

The tool, known as the ISBAR (Deteriorating Patient) sticker, is used when a patient's condition is worsening and needs to be escalated.

It helps us communicate important information about a patient's condition.

•Make sure that ISBAR stickers are accessible in your clinical area.
•Use this communication tool to share information about the deteriorating patient.
•After filling out the sticker, place it in the patient's healthcare record.

A recent audit highlighted poor compliance with this important communication tool. For more information contact beth.farren@hse.ie
